A Message from Dr. Avi Matha
At
Tamarack Hills Family Dentistry, we strive to bring the latest advancements in the
dental field to you and your family. I, along with my team, take continuing education courses and attend numerous
trainings a year that give us the skills to provide you with the treatments that make your time with us faster, safer, and
many times, more affordable.
I have completed advanced training at the
Las Vegas Institute of Advanced
Dental Studies and have taken postgraduate studies in Esthetic and
contemporary Dentistry at the
University of Minnesota. Apart from the
Cosmetic Dentistry courses, I have completed advanced Implant training and Bone grafting surgeries. I have achieved Fellowship
and Diplomate/Mastership status with the
International Dental Implant Association and fellowship status with the Academy of General Dentistry.

I am also an active member of the the
Academy of General Dentistry
, the
Practice-Based Research Network,
ICOI (International Congress of Oral Implantologist),
IDIA (International Dental Implant Association),
AACD (American Academy of Cosmetic Dentistry), and a member in the 3m focus panel group as well. These classes and organizations
help me provide a high-quality Woodbury dental home for you and your family.

Fellows and Masters of the Academy of General Dentistry
The Fellowship and Mastership designations are two of the most rigorous continuing dental education programs today. A general
dentist who is a “Fellow” in the Academy of General Dentistry (FAGD) has been recognized by other general dentists as a leader
and example to other dentists of the importance of quality continuing dental education to patient care. A general dentist
who has earned the designation “Master” of the Academy of General Dentistry (MAGD) has completed a structured and demanding
set of requirements involving all the dental disciplines.
The Academy of General Dentistry
The Academy of General Dentistry (AGD) is a professional association of more than 40,000 general dentists from the United
States and Canada. Founded in 1952, the AGD has grown to become the world’s second largest dental association and it is the
only association that exclusively represents and lobbies for the needs and interests of general dentists. AGD’s headquarters
office is located in Chicago, Ill.
Fellowship
To earn a Fellowship Award, a dentist must earn a minimum of 500 approved continuing dental education credits and pass a
comprehensive 400-question examination. The dentist is required to be an AGD member for three continuous years. While attaining
Fellowship is a challenge, it allows dentists to remain current with what is new in dentistry, and to incorporate the latest
research techniques into their practices.
How FAGD or MAGD Benefits Patients
Fellows and Masters of the AGD follow a lifelong pursuit of continuing education to provide the highest quality of dental
care to patients. The award symbolizes the professional responsibility of general dentists to remain current in their profession
and reminds each member of the dental profession to continue to learn new techniques for their dental practice. These are
awards, however and should not be confused with post-graduate degrees or certification. Every dentist who applies for the
Fellowship or Mastership Award has their application reviewed by the AGD Council on Dental Education to assure that the high
standards of the AGD are met.
How AGD Membership Benefits Patients
When a dentist has achieved the Fellowship or Mastership Award, patients can be assured that their dentist is dedicated to
providing the finest care in general dentistry. To date, more than 2276 members of the AGD have become Fellows and over 6451
have gone on to receive Mastership. The AGD is proud of all of its members who must take at least 75 hours of continuing
dental education every three years, and especially proud of its members who have made the commitment to pursue these awards.

Meet Our Team
Justine, Office Manager/Licensed Dental Assistant
As a graduate of Lakeland Medical-Dental Academy with a degree in Dental Assisting, Justine is passionate about her career
and has endless enthusiasm for all areas of dentistry. She holds over 18 years of dental experience as a Licensed Assistant,
Treatment Coordinator and has extensive training in the specialty of Orthodontics. Justine strives to make each patient’s
visit a great one! As a resident of Woodbury she and her family spend much of their time active in the community. In her
free time she enjoys spending it with her friends & family, fishing, traveling and playing Adult CoRec kickball at Bielenberg.
She is also training for her black belt in Tae Kwon Do.
Susan, Licensed Dental Hygienist/Licensed Dental Assistant
Susan graduated with an
Associates in Applied Science in Dental Assisting at Century College. Susan strives for excellency when providing care
for patients. Professionalism and integrity are her top priorities when assisting patients to ensure they have the upmost
pleasant experience at Tamarack Hills Family Dentistry. She strives to provide excellent patient care to make each patient’s
visit a memorable one. Susan is currently enrolled in
Century College’s Dental Hygiene program. In her free time, she enjoys spending time with her family, crocheting, playing
the piano and exercising.
Kelsey, Licensed Dental Hygienist
Graduated with an Associate in Applied Science in Dental Assisting and found a new passion and furthered her career in Dental
Hygiene graduating in 2016. Her greatest passion in hygiene is personally getting to know her patients. She loves visiting
historical homes and sites around Minnesota, walking at Fort Snelling and Minnehaha Falls in the Summertime, going to the
Guthrie Theatre, watching movies and spending time with her family especially the family dog Levi who has an adventurous
life swimming, canoeing, and paddle boarding!
Heidi, Licensed Dental Hygienist
Heidi graduated from Ferris State University, MI in 1992 with an Associates in Dental Hygiene. Heidi finds her job rewarding
and enjoys getting to know each every one of her patients and their oral health needs. In her free time she enjoys spending
time with her family, knitting, crocheting, sewing and walking.
Shelley, Licensed Dental Assistant
Shelley has been a dental assistant for 18 years, she strives to have patients have a great experience in the dental chair,
and excellent patient care while they are here. I love meeting new people and developing relationships with them. In my spare
time I love to spend time with my 2 boys and my grandchildren, fishing, hunting and woodworking.
Alyssa, Licensed Dental Assistant

Terra, Licensed Dental Assistant
Originally from Northern Minnesota, Terra has over 30 years as a Licensed Dental Assistant. She finds her career rewarding
and helping patients has always been her favorite part of the field. She loves spending time with family, camping up north,
fishing, and the outdoors. Terra is also a proud mama to her fur-baby Makayla, a purebred Dalmatian who will be 3 years old
in August.
Melissa, Patient Care Coordinator
Melissa has over 25 years in customer service as a Licensed Esthetician as well as a Certified Nursing Assistant. Although
new to the dental field, she has found the experience rewarding with each new day. Patient care is her number one priority.
She is a proud grandmother of 3 and loves camping at Head of the Lakes in Osakis, traveling, and doing hair in her spare
time. Her dream vacation would be lounging in the sandy beaches of Puerto Rico.
Angel, – Scheduling Coordinator
Angel is from St Paul, she is currently enrolled at Century College studying for her degree in Applied Science for Medical
Administrative Assistant at Century College. She has worked in the healthcare field since 2005 and has 6 years of Dental
Scheduling. In her free time she enjoying doing volunteer work and hanging with family and friends.
